Schedule of Fees

 

Application Fee – $1,000 (one-time)

Payable at the time the trust is funded or transferred. This fee covers the administrative costs of establishing the trust, including sub-accounts and investment setup.

 

Monthly Administrative Fee – 1.85% of Trust Assets (annualized)

Assessed monthly at the end of each month based on trust value

Fee breakdown:

• 1.10% to Enhanced Life Options Group (ELO)

• 0.75% to the financial advisor

Monthly fee equals 1.85% of trust assets divided by 12

 

Monthly Administrative Fee – Program 1 Trusts Over $1 Million

First $1 million: 1.85%

Over $1 million: .75%

Over  $5 million: .50%

The first $1 million is charged at the full 1.85% rate

 

Additional Services

Tax Returns and Legal Fees – At cost

Court Accountings – $250–$500

Case Management (when available) – $90/hour

Special Services (real estate purchase/sale, move coordination) – $60/hour

Representative Payee Services (SSI/SSDI) – $55/month (or the current SSA maximum)


Termination Fee – 3% of Trust Assets

Applied upon death of the beneficiary or transfer of the trust to another trustee

 

Special Rule for Self-Settled Pooled Trusts:

If Medicaid claims only part of the remaining assets, balance passes to heirs after 3% fee

If Medicaid claims entire balance, ELO may retain 45% of remaining assets to support disability services

 

Important Notes

• Fees are mutually exclusive

• “At cost” services are billed without markup

• Fees are subject to annual review and change
